Charlotte Crawford RN, MSN, MBA
Chief Executive Officer
Charlotte has served Lake Plains Community Care Network (LPCCN) as the CEO since July 2015. She joined LPCCN in November 2000, as the Care Management Developer and became the Executive Director in April 2006. Responsibilities over the years have included grant submissions and oversight, creating partnerships and chronic disease initiatives for the Orleans, Genesee and Wyoming Counties. In 2007, she led the initiative for LPCCN to become the Big Lake’s Emergency Medical Services Program Agency. This program initiative focuses on assisting Emergency Medical Services in Niagara, Orleans and Genesee County meet New York State requirements, provide education and other services identified by Big Lakes and the EMS agencies in the region.

Charlotte has always been devoted to community health, serving as a nurse and paramedic for over 37 years. Charlotte has been an educator in both volunteer and professional environments. She is committed to improving access to healthcare and health awareness within the community. This passion lead Charlotte to her current place of employment at Lake Plains Community Care Network. She continues to look for partnerships and ways to collaborate with others to improve the overall health in the counties we serve.

Colleen C. Kipfer
Operations Coordinator
Certified Community Health Worker

Colleen has been with Lake Plains Community Care Network, Inc. since 2002. In that time she has fulfilled the positions of Facilitated Enroller, Executive Assistant and has now, after 14 years with the company, appointed the position of Operations Coordinator this past year. Colleen has spent the better part of her adult life in the pursuit of helping others in the community’s in which she has lived. Colleen has a degree in Criminal Justice but has chosen to purse her abilities helping people in the Rural Communities of Western NY.

Patricia A. Hall
Executive Assistant

Patty has been with Lake Plains Community Care Network, Inc. since 2007 in which time she has grown and developed through four positions which include Administration Assistant, part-time EMS Program Assistant, Executive Assistant and NYS Certified Navigator. Patty has always enjoyed helping people and working with numbers. For over 30 years Patty has pursued a career in several clerical and accounting positions at various organizations throughout Monroe County, Rochester, New York. Patty has some college credits in both Business Administration and Accounting and is proficient in Microsoft Work, Excel, and QuickBooks.

Becky Boylin
Certified NYS Navigator
Program Supervisor

Becky began her journey with Lake Plains Community Care Network in January of 2001, as the Program Manager of the Facilitated Enroller Unit. The Facilitated Enrollers assisted people in applying for Medicaid, Child Health Plus and Family Health plus. At that time, our coverage area was Genesee, Orleans and Wyoming counties.

In 2013, with the inception of the New York State of Health Program, Becky became a Certified NYS Navigator helping people “navigate” the NYS Insurance Marketplace. Family Health Plus was no longer being offered but The Essential Plan and Qualified Health Plans, through the Marketplace, took its place. Our coverage area was reduced to Genesee and Orleans counties.

Becky thoroughly enjoys her position at Lake Plains Community Care Network however, she is most happy spending time with her family, her animals and working in her gardens.

Tina Wickins
Certified NYS Navigator

Tina has worked for Lake Plains Community Care Network since September of 2008. She was originally hired as a facilitated enroller for Child Health Plus, Family Health Plus, and Medicaid. In October of 2013 the facilitated enroller program ended and she became a NYS Of Health Navigator for Genesee County. Her job is to raise awareness of the NYS Of Health Marketplace in the community at outreach events, assist in applying for health insurance on the NYS of Health Marketplace, and to assist with renewals. She also helps with any problems that may arise while enrolled on the Marketplace.

When she is not working, she enjoys spending time with her husband and two children.

Kenneth Plossl
Certified NYS Navigator

Kenneth started with Lake Plains as a Facilitated Enroller in March 2012. He left to pursue other work in September 2013 but returned to us in June 2015 and was certified as a NYS Navigator. Traveling to different satellite offices throughout Orleans County keeps Kenneth very busy, but he is passionate about assisting consumers in obtaining health care and is an advocate in raising healthcare awareness. He attended school at Niagara University and got a BA in Communications Studies in 2007.

Kelly O’Neil
Certified Community Health Worker

Kelly O’Neil is a Certified Community Health Worker who was brought on staff with the inception of the Patient Activation Measure grant program, also known as PAM. In this role, Kelly works to educate and encourage community members to learn more about their health and to self-advocate for their own care. As a PAM Grant worker, Kelly strives to connect people with preventative care services, and coach them toward more efficient utilization of their Medicaid resources. She has always enjoyed working in the medical and human services fields.

Kelly is also a Certified Occupational Therapy Assistant and spent 18 years in both clinical and community settings working with people of all abilities and all ages. Kelly received her Associates of Science Degree in OTA from Genesee Community College in 1997. She enjoys spending time with her family and pets and volunteers many hours with local Fire Departments and EMS agencies, where she’s recently gained the certification of Advanced EMT. Kelly enjoys using these experiences to assist the residents of Wyoming, Genesee, and Orleans Counties toward better health and wellness as a PAM program coach.

Stephanie Salemi
Certified Community Health Worker

My name is Stephanie Salemi and I have been a Genesee County resident for the majority of my life. I graduated with my Regents Diploma and attended Genesee Community College. I recently became a Certified Community Health Worker thru LPCCN.

I enjoy working with people and assisting them in their time of need. I have learned a lot about the different services available to people in the communities including Genesee and Orleans County.

My goal is to return to college and complete my degree to further my education.

Andrew Steel EMT-P/NRP
EMS Program Coordinator

Andrew holds a degree in Computer Technology and is a graduate of Monroe Community College Paramedic Program. Prior to coming to Lake Plains Community Care Network, Inc. in 2014, Andrew was a full-time paramedic with Twin City Ambulance. Andrew started in the public safety field in 1999 and has held various positions in fire, security, dispatch, emergency medical services and volunteer sectors. Through these various positions, Andrew brings a wealth of knowledge and experience to this position. Andrew handles the day to day administrative operations as Program Agency for the Big Lakes Regional EMS Council. This includes education, distribution of policies/procedures/protocols, assistance to EMS agencies, Quality Assurance/Quality Improvement and tracking of information within the Big Lakes Region.

Andrew continues to be an active paramedic in the field which continuously brings knowledge and experience into education courses. Through the passion of helping others, Andrew has been an educator in multiple civilian, volunteer, professional and law enforcement places. Andrew holds various national and state certifications in education of which include Fire Service Instructor, DCJS Special Topics Instructor, WMD Instructor and is just completing his NYS CIC (Certified Instructor Coordinator) to name a few. Andrew continues to strive in delivering better education with continued attendance in various conferences and courses to be updated in a multitude of education principles and topics.
