Living Healthy:
Chronic Disease Self-Management Program
The Living Healthy Chronic Disease Self-Management Program is an evidence-based program that was developed at Stanford University. This program is 2.5 hours, once a week, for six weeks. Living Healthy is open to anyone in the community who is dealing with a chronic condition and wishes to enhance their ability to manage their own health care. The program is also open to family members, friends, and caregivers of those with chronic conditions. Participants will learn how to:
- Work with their Doctor
- Goal Setting
- Problem-solving
- Brainstorming
- Communication Skills
- Manage Fatigue and Frustration
- Manage Symptoms
- Relaxation Techniques
- Eat well/Develop Exercise Program
This Program is FREE*!
*Books and CD's that accompany the program are available for a small cost if participants choose to purchase them. They are not mandatory.
The program is facilitated by 2 trained volunteer leaders one or both of whom have a chronic condition themselves.
